Data Capture Form

This is a form which has been designed especially to collect data from people. Sometimes it is called a questionnaire.

The form can be either printed onto paper or presented electronically so that people can complete it and submit it online.

The form should contain a title, a brief explanation and instructions on how to complete the form. There should also be information about what to do with the form once it has been completed.
